Affordable Care Act (ACA) enhanced subsidies, created in 2021, expired at the end of December 2025. Recently, the House voted to extend these expired subsidies for ACA plans for three years. Now, the vote is in the Senate’s hands where it’s expected to be dead on arrival even though some moderate senators are working on a compromise proposal.

UnitedHealth ‘Aggressively’ Gaming Medicare Advantage, Senate Investigation Finds

Staffers for Sen. Chuck Grassley, R-Iowa, dove into 50,000 pages of documentation from UnitedHealth for the new report, which accuses the company of maintaining a workforce dedicated to upcoding.

NIOSH conducts research and issues recommendations to prevent work-related injuries and illnesses, including setting exposure limits, approving respiratory protective equipment, and overseeing support programs for first responders. Last spring, approximately 90% of the agency’s staff were terminated. Elevated utilization persisted into 2025, with payers pointing to rising hospital use, more complex behavioral health needs, increased prevalence of chronic conditions, and expanding adoption of GLP-1 drugs. Medicare remained the largest single payer, accounting for 21% of total national health expenditures in 2024.
